15. If for any reason you are unable to keep your cocker, will you agree to notify Cocker rescue immediately, and return to us without hesitation?

Please list any preference and/or concerns that you have in adopting a cocker.  I have read the above information carefully and have filled out this application honestly.  I understand that omission of information and/or failure to answer all questions and sign the application can result in this application being declined.  Also, if an omission or untruth is discovered after the adoption takes place, I understand and accept that Cocker rescue has the right to annul the adoption and reclaim the dog.

I give permission to fully investigate the information provided as well as contact veterinarians and personal contacts.  I also understand that a home and yard visit on a mutually agreed date will be done prior to final acceptance of application. 

If an adoption is agreed on, I further understand that Cocker rescue will follow up periodically to ensure that both new family and cocker are doing well together.
